Before the conversation
Use this page as a working note. Keep a copy for each option you want to compare. Mark an item only when the answer is clear to you; leave it open when you need more detail. The browser’s own print command is available if this page’s control does not work.
Write the concern in the words you would say aloud. Add the change you want help making sense of and the decision you want the visit to support. Keep any treatment name you have seen separate from that concern so the assessment has room to consider it.
Bring the medicine list and records the service requested. Ask how to share information if the visit is remote. If a support person would help you remember the conversation, ask the service how they could join. Leave space to record the names of the people involved.
Record the people and the product
Florida’s Medical Quality Assurance (MQA) search offers license, profile and disciplinary information for practitioners and establishments. The lookup process was checked September 28, 2026.
Write record details in your own notes rather than marking the item because a website displays a badge. Have the service identify its public record when the name you find does not match. Keep the mismatch open until you have an answer you can follow.
Record the cost and the next conversation
Put a note beside any box that does not fit the proposal. A cost handled by another organization still belongs in your overall comparison. An answer promised later remains open until it arrives. Keep the estimate and the written care outline with this page.
Use the answers after the visit
Read your notes while the discussion is fresh. If the answer uses words you cannot explain, ask for a plain version. You can take the same question back to the service or bring the proposal to a licensed health professional who knows your circumstances.
An aid to discussion
Do not turn a completed checklist into a score or a guarantee. Its purpose is to show what you know and what still needs discussion. Compare the quality of the answers, their fit with your concern and the practical arrangements you would rely on later.
When printing, the chapter directory and photograph are left off the paper copy so the questions have room. Keep a blank copy if you expect to compare another option. Carry the written plan and fee terms with the checklist when comparing another option.
